IRF questionnaire
Please copy and fill in the following questionnaire. Afterwards, please send in by email to iscoms@med.umcg.nl, under the subject "IRF Questionnaire". DO NOT FORGET to include personal information so that we know to which application it belongs (eg: Name, personal and/or abstract ID#, abstract titel).
The extra information from the questionnaire and previous abstracts is required simply because the research institutes and ourselves can then better evaluate any skills and expertise that you have gathered till now. Your IRF supervisor will therefore be able to adequately assess what you can still learn and see while you are in the Netherlands. It does not mean that you will not be selected if this is your first abstract, or if your answers are limited in the questionnaire. Please fill in the questionnaire as accurately as possible.
